E71: Medicare Part B Premiums, IRMAA – What Nurses Need to Know

Medicare Part B Premiums, IRMAA – What Nurses Need to Know

Many CRNAs and Nurse Practitioners spend their entire careers focused on taking care of others, assuming that once they reach age 65, Medicare will be an affordable reward for their years of service. After all, you have paid Medicare taxes on every paycheck for decades. However, many high earners are met with a shocking reality when they receive their Medicare Part B premium notice.

Instead of the standard premium, which is $202.90 per month in 2026, some nurses find themselves paying $649 per month, with their spouse facing the same amount. This results in an extra $894 per month, or more than $10,000 per year, just in Medicare Part B premiums. This happens because of a surcharge called IRMAA, or the Income-Related Monthly Adjustment Amount.

Understanding the Two-Year Look Back

The most frustrating part of IRMAA for many nurses is the timing. Medicare charges you based on the income you earned two years in the past. This means that your 2026 Medicare premiums are dictated by the 2024 tax returns you just filed.

If you are a Nurse Practitioner earning $120,000 or a CRNA making over $200,000, you are likely to hit these IRMAA thresholds. Medicare essentially treats high earners as if they haven’t paid their fair share, despite years of high stress shifts and professional dedication. Without proper planning, these surcharges can completely derail a retirement budget.

Why Burnout Changes the Calculation

For nurses, IRMAA is not just a math problem; it is a burnout problem. Many in the profession plan to retire earlier than expected because the physical and emotional demands of nursing become unsustainable. If you do not understand how IRMAA works, you might miscalculate your retirement income needs.

You might assume you only need to replace 80% of your working income, but you actually need to replace that income plus thousands of dollars in IRMAA surcharges. Alternatively, if you retired last year and your income dropped significantly, Medicare may still be penalizing you based on your peak earning years. You end up paying surcharges on income you are no longer making.

Winning the Tax Game with Strategic Levers

The good news is that IRMAA is not inevitable. There are several strategies to reduce or even eliminate these costs if you take action.

One of the most effective tools for recent retirees is the appeals process. Medicare will reconsider your determination if you have experienced a life-changing event, such as a work stoppage, divorce, or the death of a spouse. By providing documentation, such as a letter from your employer confirming your retirement, you can save thousands of dollars in premiums.

For those still in their gap years, the time after retirement but before age 73, Roth conversions can be a powerful lever. While you pay taxes on the conversion now, it reduces the size of your traditional IRAs and the required minimum distributions (RMDs) you will have to take later. This prevents a tax bomb in your 70s that would otherwise trigger permanent IRMAA surcharges.

Other strategies include making Qualified Charitable Distributions (QCDs) directly from your IRA to keep your taxable income low, or maximizing contributions to 403(b) and 457 plans while you are still working to stay below the IRMAA thresholds.

A Plan That Creates Options

Your retirement should not come with unexpected financial surprises. Comprehensive planning is essential because IRMAA is about more than just premiums; it is about how your entire income strategy works together. You deserve a retirement that reflects the dedication you have shown your patients and your community.

If you are a high-earning nurse, do not wait until you receive a shocking notice in the mail. By understanding the IRMAA brackets and planning your withdrawals today, you can ensure your money supports the life you want to live.
